Patient Rights & Responsibilities

Range of Motion, Inc. is committed to protecting the rights of every patient we serve. These rights apply to every product line we supply, whether your equipment is delivered to your home by our staff or shipped directly to you.

Your Rights

As an individual receiving home care services from Range of Motion, Inc., you have the right:

  1. To select those who provide you home care services.
  2. To be provided with legitimate identification by any person who enters your residence to provide home care for you.
  3. To receive the appropriate or prescribed service in a professional manner without discrimination relative to your age, sex, race, religion, ethnic origin, sexual preference, psychosocial state, physical or mental handicap, or personal cultural and ethnic preferences.
  4. To be promptly informed if the prescribed care or services are not within our scope, mission, or philosophy, and to be provided with transfer assistance to an appropriate organization.
  5. To be treated with friendliness, courtesy and respect by every individual representing Range of Motion, Inc., and to be free from mental, physical, sexual, and verbal abuse, neglect, and exploitation.
  6. To have your confidentiality, privacy, safety, security, and property respected at all times.
  7. To assist in the development and planning of your health care program.
  8. To be given adequate information from which you can give informed consent for the start, continuation, transfer, or termination of service.
  9. To express concerns or grievances, or recommend changes to your service, without fear of discrimination or reprisal.
  10. To request and receive complete, up-to-date information about your condition, treatment, alternative treatments, and risks, within your physician’s legal responsibilities of medical disclosure.
  11. To receive care within the scope of your care plan, promptly and professionally, while being informed of our policies, procedures, and charges.
  12. To refuse care within the boundaries set by law, and to be told what may result from that refusal.
  13. To request and receive information about services or costs privately and confidentially.
  14. To examine or review your medical records.
  15. To formulate — and have honored by all health care personnel — an advance directive such as a Living Will, a Durable Power of Attorney for Health Care, or a Do Not Resuscitate order.
  16. To expect that all information we receive is kept confidential and is not released without written consent.
  17. To review our Notice of Privacy Practices.
  18. To access, request amendment to, and receive an accounting of disclosures of your health information as permitted by law.
  19. To revoke any previous consent for release of medical information, or for media recording or filming.
  20. To be involved, as appropriate, in discussions and resolution of conflicts and ethical issues related to your care.
  21. To be informed of any experimental or investigational studies involved in your care, and to refuse them.
  22. To expect that your reports of pain will be believed, and that our staff will respond by communicating them to your treating physician.
  23. To be informed of the Medicare DMEPOS Supplier Standards, and of your option to rent or purchase certain equipment where Medicare allows a choice.
  24. To voice a complaint at any time and receive acknowledgment within 5 calendar days and written resolution within 14 calendar days.
  25. To receive this information in a manner and language you can understand, including free language assistance.

Your Responsibilities

We are partners in your care. As a patient of Range of Motion, Inc. you are responsible:

  1. To give complete and accurate information about your health, medications and allergies.
  2. To tell a staff member about your health history, including past hospitalizations, illnesses and injuries.
  3. To take part, as you are able, in developing and carrying out your service plan, including properly cleaning and storing your equipment.
  4. To review our safety materials and help maintain a safe environment in your home.
  5. To ask for more help or information about anything you do not fully understand.
  6. To notify your physician if you feel ill or have unusual symptoms.
  7. To tell us if you will not be home for a scheduled visit or delivery.
  8. To tell us before you change your address or telephone number.
  9. To tell us when you have any problem with your equipment or service.
  10. To tell us if you are hospitalized, or if your physician changes or stops your prescription.
  11. To care for the equipment supplied to you and follow your care plan.
  12. To report any concerns about pain and pain management.
  13. To show respect and consideration to our staff.
  14. To meet the financial commitments you have agreed to with us.
  15. To accept the consequences if you do not follow the proposed plan of care.
  16. To return rented equipment when your physician discontinues it, and to allow reasonable access for pickup.

How to Report a Concern

Call us first at (770) 991-4417 — we want the chance to fix it. You may also report anonymously. We acknowledge every concern within 5 calendar days and provide written resolution within 14 calendar days.

You may also contact any of these organizations directly, at any time. You do not have to complain to us first, and we will not retaliate against you for contacting them.
