THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our Obligations

Range of Motion, Inc. is required by law to maintain the privacy of your Protected Health Information (PHI), to give you this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

For Treatment. To provide, coordinate or manage your equipment and related services — for example, sharing information with your physician about the item ordered for you.

For Payment. To bill and collect payment — submitting claims to Medicare or your insurer, verifying eligibility, and obtaining prior authorization.

For Health Care Operations. For quality review, training, licensing, accreditation (including The Joint Commission), auditing and general business operations.

Delivery and Service Communications. To contact you about deliveries, pickups, equipment checks or supplies related to items you already receive from us.

Individuals Involved in Your Care. With your agreement, or when otherwise appropriate, we may share relevant information with a family member or caregiver involved in your care.

As Required by Law. Including to the Secretary of the U.S. Department of Health and Human Services when required to investigate our compliance.

Other Permitted Disclosures. As permitted by law: public health activities; health oversight (audits, inspections, licensure); judicial and administrative proceedings; law enforcement purposes; coroners, medical examiners and funeral directors; organ donation; approved research; workers’ compensation; military and national security; and to avert a serious threat to health or safety.

Uses that always require your written authorization. We will not use or disclose your information for marketing, we will not sell your information, and we will not disclose psychotherapy notes, without your written authorization. You may revoke that authorization in writing at any time.

Your Rights

  • Inspect and copy your health information, including an electronic copy where we hold it electronically. A reasonable cost-based fee may apply.
  • Request an amendment if you believe information is incorrect or incomplete. We may deny the request in certain cases and will tell you why in writing.
  • Receive an accounting of certain disclosures other than for treatment, payment and operations.
  • Request restrictions on how we use or disclose your information. We are not required to agree — except that we must honor a request not to disclose information to your health plan about a service you paid for in full, out of pocket.
  • Request confidential communications at an alternative address or by an alternative method.
  • Receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.
  • Be notified of a breach of your unsecured protected health information.

Changes to This Notice

We may change this Notice and make the revised Notice effective for information we already hold. The current Notice is posted at our facility, provided in your patient packet, and available on this page.

Complaints

If you believe your privacy rights have been violated, contact our Privacy Officer, or file a complaint with the U.S. Department of Health and Human Services. You will not be penalized in any way for filing a complaint.
