COMMON BILLING QUESTIONS
What is an EOB? An EOB, or Explanation of Benefits, is the statement your insurance company sends you after we bill them for your equipment. It shows what we charged, what your plan allowed, what your plan paid, and what portion — if any — is left for you.
An EOB is not a bill. You do not need to send payment when you receive one. If you do owe something, we will send you a separate bill, and the amount should match the "patient responsibility" line on your EOB. If the two do not match, call us at (770) 991-4417 before you pay anything.

What are deductibles, coinsurance, and copays? These are the three ways an insurance plan asks you to share the cost of your care.
- Deductible — the amount you pay each year before your plan begins paying. If your deductible is $1,500, you pay the first $1,500 of covered costs yourself.
- Coinsurance — a percentage of the cost that you keep paying after your deductible is met. Under Original Medicare Part B this is 20%: Medicare pays 80% of the approved amount and you pay the other 20%.
- Copay — a flat dollar amount for a specific item or visit, such as $30, no matter what the total cost is.
Most commercial and Medicare Advantage plans also have an out-of-pocket maximum. Once you reach it, the plan pays 100% of covered costs for the rest of the plan year. Original Medicare does not have an out-of-pocket maximum.

COMMON CPM QUESTIONS
When will you deliver my CPM machine? We typically will call you 48-72hrs prior to your surgery to schedule delivery either prior to your procedure or within 48hrs of your return home.
- Your CPM will need to be set up on a large flat surface (such as a bed)
- The patient will need to be present, CPM is custom fit to patient’s leg length
- It is best that a caregiver is also present to review instructions and address any potential trip hazards that were identified
When will you pick up my CPM machine? We will call you on your 20th or 21st day of use. Please note that we will not continue to bill you or your insurance past the 21st day even if we have not yet picked up your machine.
What do I need to do to return my CPM machine? Please call us at 770.991.4417 if you would like for us to pick up your machine prior to the 21st day of use. Please remove all soft goods (the sheepskin cover) as those are yours to keep.
