Insurance and Assignment
Range of Motion, Inc. is in network with, or holds contracts with, a broad range of federal, private and workers’ compensation plans, and we are an accredited Medicare DMEPOS supplier. We accept Medicare assignment for the items we bill to Medicare — we accept the Medicare-approved amount as payment in full, and you are responsible only for any unmet deductible and applicable coinsurance.
We make every attempt to verify your benefits before or by the date your service is rendered. Verification or authorization is not a guarantee of payment; it allows us to give you an estimate. Your responsibility may change based on final processing of the claim by your insurer, so you may receive an additional bill afterwards. You have the right to accept or decline at the time services are rendered.
Renting versus Buying
For inexpensive or routinely purchased equipment, you may choose to rent or to purchase. For capped rental equipment, Medicare pays a monthly rental for up to 13 months of continuous medical need, after which ownership transfers to you. We tell you which category your item falls into before service begins, and it is stated on the product insert supplied with your equipment.
If Medicare May Not Pay
If we believe Medicare is unlikely to pay for an item, we will give you an Advance Beneficiary Notice of Non-coverage (Form CMS-R-131) before you receive it. That notice explains why we expect a denial and what the item may cost you, and lets you decide whether to accept financial responsibility. You will never be billed for a non-covered item without first receiving and signing that notice.
Workers’ Compensation
Please provide the claim adjuster name and number, your claim number and date of injury so we can obtain approval. Services not authorized are the patient’s responsibility.
Payment
We accept Medicare, third-party insurance, Visa, MasterCard, ATM/debit cards, checks, money orders and cash. We do not accept Medicaid. Returned check fee: $30.00, or the state maximum if less.
